Abstract
Grand River Hospital (GRH) deployed an innovative patient portal application in July 2004 for patients diagnosed with breast cancer called My Care Source. (see:https:mycaresource.grandriverhospital.on.ca). This session will provide an overview of how Grand River Hospital has exponentially expanded the depth and breadth of the application across the cancer continuum maximizing connectivity to health information systems. It will illustrate how GRH has taken the lessons learned from the first iterative process to move forward to develop the patient portal together with the University Health Network and the Hamilton Niagara Haldimand Brant LHIN for patients diagnosed with Chronic Kidney Disease. This project, initiated in December 2006, is supported by Canada Health Infoway as part of its Innovation and Adoption Program.
About the Speaker
Claudette DeLenardo RN MSN is the Director, e-Health Technologies at the Grand River Hospital, Kitchener, Ontario. Claudette obtained her Bachelor of Science in Nursing from Laurentian University and Master of Science in Nursing at the University of Phoenix. She has worked in health care in both acute and non-acute settings and has been involved in the development and application of patient portal technologies since 2003. Claudette is a member of the Canadian College of Health Service Executives and is actively involved in the Registered Nurses Association of Ontario. Claudette is a recipient of a 2004 Healthcare Informatics, IT Innovator Award and 2007 Cancer Quality Council of Ontario Individual Innovation Award.
