Description	This article paints a picture of a Canadian public health care system on an unsustainable course, a course that will plunge Canada's Medicare system into ever-deepening crises over the next 10 to 20 years. The roots of the system's difficulties are complex and intertwined. Important roles are played by a number of design flaws in Medicare itself, as well as by the changing circumstances in which Medicare operates. A key focus of the article is on the need for outcome assessment and on some of the reasons why this is not yet done.
