Program Changes and Course Cancellation
The National Institutes of Health Informatics (NIHI) reserves the right to: limit registration in this course; to make changes to any printed or online information on programs, instructors and course information; and to cancel this course for any reason, including enrolment does not reach the required minimum, or for other unavoidable factors. If this course is cancelled, all participants will be contacted via e-mail, telephone, or post mail before the course begins, and registration fees will be reimbursed in full.
Registration cancellations after September 15, 2012 are non-refundable.
All cancellations must be submitted in writing or via email to the NIHI Registrar at movenden AT nihi.ca.
Cancellations prior to 5:00 PM EDT, September 15, 2012 will be eligible for a full refund less $100 of the registration fee paid and a $50 administration fee.
An alternate attendee name may be substituted at any time through the online registration system or by emailing the NIHI registrar at movenden AT nihi.ca. Please supply name, position, email and phone number for the alternate attendee.
Credit Cards that need to be reprocessed are subject to a $75 administration fee.
All refunds will be processed within 7 weeks after the close of the course. The refund will be made via cheque. Participants will be requested to allow a reasonable amount of time for the refund to be processed.
No postdated cheques are accepted.
There is a $50 fee for NSF cheques.
